Aberrant expression of double stranded RNA dependent protein kinase in hepatocytes of chronic hepatitis and differentiated hepatocellular carcinoma. Using a monoclonal antibody, 71/10, we immunohisto-chemically analyzed the PKR expression in 64 human liver biopsies and 25 liver sections of rats inoculated with diethylnitrosamine (DEN). The PKR expression in fatty and normal livers was minimum. Scoring signals from 0-4, the average scores of chronic 14-active (cah) and 6-persistent HCV-hepatitis (cph) were 2.8 and 2.0 (P = 0.038). For both cah and cph, the stained cells were more abundant in the periportal than in the centrilobular regions (P < 0.001). HCV liver cirrhosis also expressed PKR. The average scores of well-, moderately- and poorly-differentiated HCV-hepatocellular carcinomas (hcc) were 3.4, 2.1, and 0.3, respectively (P < 0.001 each). Those of well- and poorly-differentiated HBV hcc were 2.3 and 0.0, respectively. Those of rat hcc 5, 15, and 25 weeks after the last dose of DEN, 1.8, 2.0 and 2.0, were significantly higher than those of adenomatous hyperplasia, 0.9, 0.8 and 0.4, respectively. Most PKR-positive cells had integrated nuclei, and the PKR-index was approximately 20 and 100 times higher than the apoptotic index in HCV hepatitis and hcc, respectively.
