Results of video-assisted lobectomy for lung cancer: Tsuguo NARUKE, Ryosuke TSUCHIYA, Haruhiko KONDO, Haruhiko NAKAYAMA, Hisao ASAMURA (Div. of Thoracic Surgery, Natl. Cancer Ctr. Hosp.)
79 cases of VATS lobectomy for Lung cancer have been performed at the National Cancer Center Hospital during the last 5 years. Within these cases, the total 17 conversions were made under the following circumstances: 4 for lymph node metastasis, 3 for pleural adhesion, 3 for bleeding, 2 for lowering O2 saturation, 2 for autosuture trouble, one for severe inflammatory hilar node, another one case where the lymph node dissection was not possible, and the last one where one lung ventilation was not possible. The primary site of the total 62 VATS lobectomy cases were as follows: 26 in the right upper lobectomy, 4 in the right middle lobectomy, 15 in the right lower lobectomy, 10 in the left upper lobectomy, 6 in the left lower lobectomy, and one in the right and lower lobectomy. The pathological stage classification of these 62 VATS lobectomy cases were, 45 of T1N0M0, 15 of T2N0M0, and one each case of T4N0M0 and T2N0M1 respectively. Regarding the 17 converted cases, the pathological stage classification were, 9 of T1N0M0, 5 of T2N0M0, and one each case of T3N0M0, T1N2M0, and T1N2M1 respectively. Concerning the prognosis, 2 out of 62 VATS lobectomy patients and 2 out of 17 converted patients died within 3 years which can be proved by the following figures of the survival rate: 94.1% in VATS lobectomy and 75.8% in conversion. Furthermore, the 45 cases out of 62 VATS lobectomy were proved to be alive with no evidence of diseases. Early results indicate that video thoracoscopic lobectomy is a safe and effective procedure for selected pripheral type of stage 1 lung cancer. Further evaluation and prospective studies are indicated.
